Some combos specify upper attacks and some lower. For upper, I assume I can hit the upper arrow keys but there are no lower arrow keys for Ranger as there are for melee classes. How do I make lower attack?

You do get lower attacks, check abilities. If not u probably will get that with higher level.
Hit level 40. Lower attacks are not available before that point.
