I'm using this for everything except Entity:

Somehow feating Eye of Penance allows you to spam anything you want.
For Entity I think it's better to run something like this:

It's heavily butchered since all your close range spells won't work anyway. Extra prot and hp will let you keep more dps gear on if you're standing on the ledges.
